Black-box decisions create new risk.
Unlike other failure patterns, black-box behavior introduces risks that do not exist in manual processes. Manual processes may be slow or inconsistent, but they are explainable. A person can describe why a decision was made, even if the reasoning was imperfect. Black-box automation removes that fallback.

If the system cannot answer these questions in business terms, teams cannot defend its outputs. At that point, automation becomes informationally weaker than a junior staff member.
